The Waterloo Girls Minor Hockey Member Agreement

Legal Member Agreement To Participate In The Programs of the Waterloo Girls Minor Hockey Association.

As a participant/parent, I agree to be subject to the rules, regulations, and decisions of the LLFHL, OWHA, CHA, and the Waterloo Girls Minor Hockey Association and its executive which may restrict in some areas such as movement from team to team, conduct, etc. and agree to abide by such rules, regulations and decisions of the LLFHL, OWHA, CHA, and the Waterloo Girls Minor Hockey Association and its executive.

I am aware that these rules and regulations are available to me through my local executive and certify that the provided information is true.

Waiver of Liability

For Waterloo Girls Minor Hockey Association and all of its directors, organizers and members:

I hereby indemnify and save harmless Waterloo Girls Minor Hockey Association, hereby known as the WGMHA and including all of its directors, members, non-member players and affiliated associations from and against any and all claims by any person or persons arising out of any act or neglect in respect to any thing associated with any and all activity that is in any way affiliated with the WGMHA. This includes but is not limited to any injuries sustained before, during and after any hockey game/practice and related activity.

This waiver of liability will include travel time, before and after any related hockey activity and will again indemnify and save harmless the WGMHA, regardless of any negligence of the WGMHA.

I hereby agree to all of the above conditions stated in both the Code of Conduct and Waiver of Liability. I agree and accept the above conditions on behalf of all family members. I agree not to bring any legal action against the WGMHA for any reason whatsoever.
