Any player currently registered with the Waterloo Girls Minor Hockey Association is required to obtain a Permission to Skate form to attend tryouts with another
association.

Request for Permission to Skate Form must be forwarded by email to
Robert Hennig, President WGMHA at ravensregistration@rogers.com stating the following:

• Player's full Name
• Team and Association and Category for which she is trying out for
• Time Frame Required

(A separate Permission to Skate Form is required for each team the player is trying out for.)

Requests for Player Release

Any player currently registered with the Waterloo Girls Minor Hockey Association requiring a Player Release form must forward their request by email to
Robert Hennig, President WGMHA at ravensregistration@rogers.com stating the following:

• Player's full Name
• Date of Birth
• Intended Team and Association
• Time Frame Required

All requests for Player Releases must be approved by a quorum of the WGMHA Executive . Please allow sufficient time for your request to be approved.

You do not need a Player Release or a Permission to Skate form for the following

• Joining or trying out for a different team within the same organization
• Moving from boys’ hockey to girls’ hockey
• Moving from girls’ hockey to boys’ hockey
